Hello! It is almost time for your second dose of COVID vaccine! Please use this link to book your second appointment as close to 21 days from your first dose as possible. We understand that you may have been told to come back 21 days to the day for your second dose. However, owing to distribution and delivery schedules, we may not have second doses available exactly 21 days from your first dose. Not to worry! There is a 4-day grace period, so any of the slots this week should work for you.
Please do not make multiple appointments. Your email confirmation will arrive in your inbox within two hours, however, your appointment is confirmed as soon as you see the page titled “COVID Vaccine Second Dose Confirmation.” If your email doesn’t arrive, don’t worry, it could be in your spam, your inbox may be full, or you possibly entered an incorrect email. As long as you reach the confirmation page, you are booked.
We will ask you the following questions on the day of your appointment. Please be advised that if you answer yes to any of these questions or if you are sick or in quarantine on the day of your appointment, you will be required to reschedule.
- Are you sick today? (Fever, cough, shortness of breath, nausea/vomiting in the last 24 hours)
- Have you received antibody therapy or convalescent plasma for COVID treatment in the past 90 days?
- Have you received a vaccine in the past 14 days?
- Are you currently in your isolation or quarantine period due to COVID-19?

From: $50.00To book an appointment, first select the date on the calendar, then the time slot below. Then use the arrows to select the number of travelers. If you need to make an appointment for more than 6 people, please call us to book.
Appointments cost $100 for an individual traveler and $50 per additional person with the same itinerary. A 50% deposit is required at the time of booking. This deposit is refundable until 7 days prior to your appointment. Cancellations made within 7 days of the appointment are non-refundable. The balance will be collected at the end of the appointment.
Costs for vaccines and medications are NOT included in the fee. While we make every effort to bill each patient’s insurance plan, the patient may be responsible for paying all or part of the cost of the vaccines and medications.
A link to a pre-travel history form will be emailed to you after you book your appointment. This form must be filled out for EACH traveler (the link may be used more than once). This form must be completed at least 7 days prior to your appointment to ensure adequate time for our team to prepare for your visit. Failure to complete this form on time may result in the cancellation or rescheduling of your appointment.
