COVID Vaccine | Deerfield Location Booster Dose

To book an appointment, first select the date on the calendar, then the time slot below. Then use the arrows to select the number of travelers. If you need to make an appointment for more than 6 people, please call us to book.

Appointments cost $100 for an individual traveler and $50 per additional person with the same itinerary. A 50% deposit is required at the time of booking. This deposit is refundable until 7 days prior to your appointment. Cancellations made within 7 days of the appointment are non-refundable. The balance will be collected at the end of the appointment.

Costs for vaccines and medications are NOT included in the fee. While we make every effort to bill each patient’s insurance plan, the patient may be responsible for paying all or part of the cost of the vaccines and medications.

A link to a pre-travel history form will be emailed to you after you book your appointment. This form must be filled out for EACH traveler (the link may be used more than once). This form must be completed at least 7 days prior to your appointment to ensure adequate time for our team to prepare for your visit. Failure to complete this form on time may result in the cancellation or rescheduling of your appointment.
