COVID Vaccine | McFarland Location Week of June 7 P2H
When booking, please use the name and date of birth for the person getting the vaccine. You will need to book TWO (2) appointments. The calendar displayed is for your first dose. After you select a time slot, you will check out and be sent to another calendar to book your second dose three weeks (21 days) from your first dose. Please note that if your second dose is due in the next month, you will need to advance the calendar. Your booking will not be complete until your first and second doses are booked. This is the Pfizer vaccine, which is approved for individuals 12 years of age and older.
- Please bring your driver’s license and prescription insurance card
- Fill out the intake form. The intake form will appear on the screen after you book your time. Your booking will NOT be complete until this form is filled out.
We will ask you the following questions on the day of your appointment. Please be advised that if you answer yes to any of these questions or if you are sick or in quarantine on the day of your appointment, you will be required to reschedule.
- Are you sick today? (Fever, cough, shortness of breath, nausea/vomiting in the last 24 hours)
- Have you received antibody therapy or convalescent plasma for COVID treatment in the past 90 days?
- Have you received a vaccine in the past 14 days?
- Are you currently in your isolation or quarantine period due to COVID-19?
Please do not call or email the stores if appointment slots are no longer available.
If you cannot click on the calendar, that means all appointments have been reserved. All new booking information will come via email.

Travel Clinic Appointment
From: $50.00To book an appointment, first select the date on the calendar, then the time slot below. Then use the arrows to select the number of travelers. If you need to make an appointment for more than 6 people, please call us to book.
Appointments cost $100 for an individual traveler and $50 per additional person with the same itinerary. A 50% deposit is required at the time of booking. This deposit is refundable until 7 days prior to your appointment. Cancellations made within 7 days of the appointment are non-refundable. The balance will be collected at the end of the appointment.
Costs for vaccines and medications are NOT included in the fee. While we make every effort to bill each patient’s insurance plan, the patient may be responsible for paying all or part of the cost of the vaccines and medications.
A link to a pre-travel history form will be emailed to you after you book your appointment. This form must be filled out for EACH traveler (the link may be used more than once). This form must be completed at least 7 days prior to your appointment to ensure adequate time for our team to prepare for your visit. Failure to complete this form on time may result in the cancellation or rescheduling of your appointment.
