Sam Lenzendorf, PharmD

Sam brings nearly a decade of pharmacy expertise to her role as a float pharmacist with Forward. A proud graduate of Midwestern University Chicago College of Pharmacy, she discovered her passion for patient care at just 15 years old—a calling that continues to energize her work today.

While she grew up in the suburbs of Chicago, Sam has deep Wisconsin ties—her parents hail from Theresa and Sparta — and ensured that she was a Packers, Badgers, and Brewers fan from the start. 

Sam is committed to delivering the reliable, personalized care that builds trust between pharmacist and patient. She brings both clinical excellence and genuine care to every interaction, helping improve health outcomes in the communities she’s proud to call home.
